Lost my Prescription

When you have a sight test, you should be issued a copy of your prescription by your optician, whether you opt to choose your specs there or not. Your prescription is part of your eye test, and your entitlement, whether you paid for your eye test, it was NHS funded, or you received it as an offer.

Reasons for asking for a copy:

You may not have been issued it in the first place.
You may have been in a rush to leave, and forgot to collect it on your way out.
The optician may have offered to keep it for you for your return.
Even a technical glitch can sometimes mean it mightn't have been issued on the day, e.g. the printers were down.
You've mislaid it - in an ever increasingly paperless society, we've found this to be the most common case.
In a world of standing orders, direct debits, and electronic reminders, it's easy for the odd paper document to go a stray,
this however still entitles you to ask for a replacement copy, and your optician will be able to make a copy for you.

Your options for getting a copy of your prescription:

Phoning - Some opticians may after verifying your identity over the phone, be happy to send you a copy by email or post
Visiting the shop - You can be issued a replacement there and then.
Emailing, faxing, posting - if you send a written request, your optician will be able to reply with your prescription over email, fax or post.

MAKING IT EASY
We've created a generic form for download, in case you want to contact your optician from the comfort of your own home.
Simply fill in the blanks and send it off to them.
